Comparative analysis of morphometric parameters of the portal vein roots in adult men with different somatotypes and body types

Abstract

Introduction.The study of the morphometric characteristics of the portal vein roots from the standpoint of a constitutional approach is an urgent area of clinical anatomy. The use of somatotypological techniques by M.V. Chernorutsky and Hit–Carter allows taking into account individual body features in the analysis of vascular anatomy. The use of multispiral computed tomography ensures high accuracy and objectivity of the morphometric assessment of the portal vein system.The aimis to study the morphometric parameters of the portal vein roots in mature and elderly men with different somatotypes and body shapes.Materials and methods.66 men who underwent multispiral computed tomography were examined. A morphometric assessment of the superior, inferior mesenteric and splenic veins was performed; the body shape was determined by the method of M.V. Chernorutsky (Pinier index) and Hit–Carter.Results.In 66 of the subjects, Chernorutsky’s body shapes generally corresponded to Heath–Carter somatotypes, while the latter technique revealed intermediate variants and a wider variability of anthropometric indicators. In 86.5% of cases, the trunk of the portal vein was formed due to the fusion of the superior mesenteric and splenic-mesenteric trunk. Vascular diameters were larger in hypersthenics and meso-endomorphs and smaller in asthenics and ectomorphs, while the largest vein length was observed in individuals with a pronounced ectomorphic component. A pronounced dependence of the morphometric characteristics of the portal vein roots on the human somatotype has been established. Hypersthenic (meso-endomorphic) variants are characterized by increased diameters and relatively shorter vein lengths, whereas asthenic (ectomorphic) somatotypes are characterized by a smaller vascular lumen with a larger length.Conclusion.The morphometric parameters of the portal vein tributaries demonstrate a pronounced somatotype-related variability: the hypersthenic (meso-endomorphic) constitution is characterized by larger vessel diameters and shorter venous trunks, whereas the asthenic (ectomorphic) somatotype is associated with a smaller luminal diameter and greater vessel length — findings that should be taken into account in clinical and surgical practice when planning interventions on the portal venous system.
