Микробиота легких при туберкулезе — практическое значение (обзор литературы)

Аннотация
Туберкулез остается одной из ведущих причин смертности от инфекционных заболеваний во всем мире. На сегодняшний день достигнут определенный прогресс в борьбе с этим тяжелым заболеванием, но такие проблемы, как множественная лекарственная устойчивость микобактерий туберкулеза и коинфекции туберкулез/ВИЧ, вызывают серьезную обеспокоенность. Значимым открытием стало обнаружение микробиоты в дистальных отделах дыхательного тракта — респираторных бронхиолах и альвеолах, которая, как выяснилось, играет важную роль в поддержании местного иммунитета и защите от патогенов. У здоровых людей в этом локусе доминируют комменсалы, напримерPrevotella, тогда как при туберкулезе развивается дисбиоз: снижается видовое разнообразие и увеличивается численность условно-патогенных микроорганизмов (Streptococcus, Neisseria). Этот специфический профиль микробиоты может служить диагностическим и прогностическим маркером, помогая выявлять болезнь на ранней стадии и прогнозировать риск развития лекарственной устойчивости микобактерий туберкулеза. Противотуберкулезная терапия при этом восстанавливает нормальный состав микробиоты легких, но одновременно способствует росту антибиотикорезистентности среди условно-патогенных бактерий. Перспективным направлением является управление микробиотой легких. Разработка ингаляционных пробиотиков открывает новые возможности для профилактики туберкулеза и повышения эффективности лечения, особенно в сложных случаях множественной лекарственной устойчивости микобактерий туберкулеза. Таким образом, изучение микробиоты легких знаменует переход к персонализированным подходам в борьбе с туберкулезом.
