Значение и принципы применения эндоваскулярных технологий и «гибридных подходов» в лечении окклюзионных поражений артерий нижних конечностей
НОВЫЕ ТЕХНОЛОГИИ В МЕДИЦИНЕ
Аннотация
Прежде чем принять решение о терапевтическом или хирургическом лечении, врач должен понять естественную историю облитерирующего атеросклероза и хронической угрожающей конечности ишемии. Очевидно, что легкая ишемия, проявляющаяся хромотой, не подвергает пациента значительному риску потери конечности. Анализ литературы показывает, что в настоящее время все чаще применяются гибридные и эндоваскулярные подходы, являющиеся альтернативой традиционным операциям. Большинство исследователей доказали, что при выполнении «гибридных» и эндоваскулярных операций очень низка частота сердечно-сосудистых проблем и инфекционных осложнений, а в большинстве случаев удается сохранить конечность. Однако принцип и значение эффективности того или иного метода либо подхода к лечению многоуровневых облитерирующих атеросклеротических поражений артерий нижних конечностей зависит от степени ишемии конечности, а также от анатомического уровня поражения, на котором выполняется реваскуляризация. Мы уже опубликовали множество статей о концепции, преимуществах, сравнительных характеристиках гибридного подхода в российских журналах высокой аттестации. В данной обзорной статье анализируются и обобщаются возможности «гибридного» и эндоваскулярного подходов в лечении пациентов с многоуровневыми облитерирующими атеросклеротическими поражениями, стенозами и хронической угрожающей ишемией конечности.
